Summary: Kill premix channel

Initial Comment:
This patch removes the premix channel and changes all uses of it to a regular permanent channel.

I did a quick test with the few games I have with me, and both Sword2 and the mod player still function.

The patch is fairly trivial and, barring any grand thinkos, seems low-risk to me.

There's a comment above Mixer::isReady() in mixer.h that still mentions the premix channel, but I don't know enough about the situation it mentions to alter it appropriatly for a premix-free world.
