Summary: Midi device selection through commandline in windows

Initial Comment:
This patch allows for manually selecting a midi device through commandline parameter --midi-device=device which is only available in windows.
This removes the need for users modifing the global midi mapper in XP + earlier, or using third party tools in vista + later. If the parameter is not defined by the user, the Midi mapper is used as before.

The next step is probably to add GUI control for this parameter.
