README: Fix dmedia env var description; tweaked some wording

The IRIX dmedia driver documentation incorrectly asked the user
to use SCUMMVM_PORT to override the sequencer port, but the
driver actually checks SCUMMVM_MIDIPORT.

Also tried to clarify some MIDI related wording
