Summary: FluidSynth too loud

Initial Comment:
I've heard reports that the FluidSynth MIDI driver is
too loud. This probably depends on which SoundFont is
used. With the one I tried, the default volume was much
too low, so I cranked up the "synth.gain" setting from
its default (0.2, I believe) to 2.1. I'm not sure how
gain is defined, but it might be dB.

So perhaps we should make this configurable, but I'm
not sure what the best way to handle floating-point
settings is.

The attached patch is completely untested, and might
not even compile. I've never managed to compile
FluidSynth under MinGW.
