Initial Comment:
When patching the working CD v1.0 with v1.2 from http://www.spacequest.net/sq4/patches/ then it sometimes won't launch, displaying:

When it does launch, it got crashed after seeing the slime in the sewer (attached).

Note that v1.2 is supposed to have been offered by Sierra itself. It fixes important bugs like the inability to have text and speech at the same time.
