Summary: add support for Dutch/Mac version of Balloon-O-Rama

Initial Comment:
The Dutch/Mac version of Balloon-O-Rama has a zero padded index file. This patch adds the filename pattern for detection, md5 and a small fix to prevent scummvm from exiting on reading the weird index. This is just one way of fixing it, another could be assigning the game it's own GID and check for that in addition to the itemsize = 0. Feel free to alter the patch to use the preferred approach.
