 I may be able to answer that one. The game writes completion state to a
 config file, because otherwise it has no way of knowing on (e.g.) a
 Sorceress run that you've completed a Warrior run some other time that
 you've played the game.

 So if this config file cannot be written (because of your user rights
 settings in Windows) then the achievement cannot unlock. This is pretty
 rare though, considering how rights are usually set up in Windows.

 This is not a ScummVM bug, it applies to the Steam version as well.
