 (1), (3), (6), and (7) will all be fixed by the fix for #16732 which I
 will merge soon.

 As for the sfx/speech/midi volume issue, the engine does not distinguish
 between sfx and speech so the volume control for sfx controls both.  The
 midi slider needs fixing though.
