Summary: MT-32 instrument definitions

Initial Comment:
Here is some entries for the MT-32 custom instrument
translation table. These seem to be the ones most used
in monkey2.
I had to make one slight change to the mt32-gm table as
well in order to use GM instrument 95 (Sweep pad) for
Swell 1 and Swell 2
